Job Details:

Fremont Health has an exciting full time opportunity for a Hospice RN.
The Hospice RN will be responsible for managing assigned caseloads, assess client needs, develop an interdisciplinary plan of care with client/family participation as well as organize and coordinate care with appropriate referral to internal and external resources.
Minimum qualifications include:

Current RN licensure by the State of Nebraska.

Graduate from an accredited Diploma School of Nursing/College of Nursing with an ASN or BSN required;Bachelor s Degree preferred.

Knowledge of computer resources.

Proficiency in nursing practice

Exceptional ability to be self-directed.

Outstanding ability to communicate with a wide variety of disciplines.

Valid drivers license and current automobile insurance.

Experience is highly preferred.
